Connections Count
Are you the parent or caregiver of a child between the ages of 0 and 5 years old who needs some extra support in accessing resources?
The Connections Count Program at Lakeshore CAP works with families in Manitowoc and Sheboygan Counties who are expecting and/or caring for at least one child under the age of 5. This 20-week program is designed to work with families by helping them find the community resources, services, supports and opportunities that will address each family’s identified needs and goals.
The 3-step program includes:
- Accessing your strengths and needs
- Developing your family goal plan
- Providing information, community resources and supports
Following a community partner referral or self-referral to the program, a Community Connector will meet with each family to determine if the program is a good fit. The Connector works closely with you during this process to identify your strengths and needs and develop a goal plan. We also provide information, resources and support unique to your family’s concerns.
